Evidence-Based Initiatives

Blood Pressure Self-Monitoring Program (BPSM)

“At the Y, we’re committed to supporting the health of our community, especially those at risk for chronic conditions like high blood pressure. That’s why we offer the Blood Pressure Self-Monitoring Program—a simple, supportive way to help people take control of their heart health. Participants work with trained Healthy Heart Ambassadors to track their blood pressure at home and receive personalized guidance, all in a welcoming, non-clinical setting. This program isn’t just about numbers—it’s about helping people live longer, healthier lives. It’s one more way the Y is going beyond the gym to make a real impact.”
